Default Slow acceleration after idling

So I've got my '04 XA up to 215,000 miles. No major issues other than maintenance type work.


The last few weeks when I'm stopped at a light and then try to accelerate, it feels like something is keeping it from moving for about 5 seconds and then it takes off. It sounds/feels like I am accelerating and taking my foot off the gas quick then accelerating again when I'm not if that makes sense.


It's also hiccupping every 15 seconds or so like there is a misfire when idling.


Finally, if I have to stop on an incline, same thing - it has barely any power to get up the hill but then it kicks in and I take off.


Any ideas before I take it in for a look? I'm hoping it might be something easy like spark plugs since I had a similar issue in another car and that took care of it. I had them replaced about a year and a half ago in this one.
