Default Rotor/Axle/ maybe CV joint?

Need some help, after brake change there started a clicking noise from the passenger side, looked over the net and in forums could not find anything about it only stuff about CV joint but I dont think that is causing this because when i took the wheel off I noticed that my rotor moved and clicked (on lugs) so I figured I didnt tighten the lugs enough so I double checked the torque and it was fine? the clicking noise is when I brake forward and reverse, and when I go over small bumps, I can even make it click on command (tapping the breaks repeatedly)

Here are some pick of how off the rotor looks and a video of the sound made by hand.

Toyota are bad to have a lot of "pad shift" when going over bumps and shifting from reverse to drive if the fit kit for the pads are not used. basically the pads are shifting in the caliper and are usually called a clicking noise. did you reuse the factory fit kit for the pads?

I didn't see any type of kit when I replaced the brake pads. I know I didn't replace the hardware but I am purchasing a set to see if that helps.

By kit what entails in the kit.

there is a little threaded hole on the rotor you can see in the pics. take off the rotor and line that hole up with the hole in the hub. then get the bolt that goes in there from the dealership.

I noticed that about the rotors when I did the tire rotation. The rotors are positioned just like the pictures too. It doesn't surprise me that it wasn't mentioned when the car went in for it's first two free maintenance at the dealer. Will have to look into the bolt to keep the rotor in place.

ummm if I remember correctly its basically just a shim that goes on the outside of the pads, and possibly some type of adhesive for it.....last I checked about 13.00 from the dealer
