and as far as prices go, i get a hook up cause im really good friends with the owners of the shop, i got 200 in it so far, ill have about 500 once its done, that side piece i got was right at 150, and it pretty good size, but i tip big, so hook up or not ill have just over 600 in it

really it would depend on the artist, some charge more to draw something up because it's custom and takes time. some charge extra for custom pieces period, others charge just by location and size of the tat(meaning the drawing up is actually free). find a good artist and ask them, how they would charge you if it's custom.
i would think many artist may prefer drawing it up, b/c it can add to their rep. i wouldn't go to someone that couldn't draw; so if i saw someone with a nice piece and they tell me their tattoo guy/girl drew it up, then hands down i'm checking out that person over the one that just copies from the stickys. it's more time consuming, but it can end up bring in more customers in the long run, and probably keep you coming back.

You people have inspired me. I finally figured out what I want to get for my tattoo...
