1) Yes. You can build a mounting plate.
2) I went underneath the door sills on both sides of the car.
3) There is a rubber grommet right behind the driver's side strut that I went through.
4) The speakers in the front door are riveted. Just drill through the rivets and they'll pop out. If you have a double DIN head unit, you'll need some side pieces because the Toyota headunit is about an 1" wider than DIN. There are tons of single DIN dash kits. Other than that, it's incredibly easy to rip apart an xB.

2. I ran wiring to my amp under the passenger seat by going under the carpet. However, going under the plastics bordering the doors/lower B pillar is easy as pie. Just pop it off, stick your wire in there, and snap it back in.
3. Uncompiled's got it right. please slit in the grommet and insert your wire that way. Do not pull the grommet back and stick the wire in along side it. You will be unhappy when your wire chafes to bare metal and hits the body of the firewall and causes a short.
4. It's really easy to take apart. with a cheap socket set, a screwdriver, and some patience, I think I could have the entire inside of the car removed and I'm a moron when it comes to cars. If it isn't screwed in, it pops off. if it's screwed in, unscrew it. the only exception is the front door speakers.

What they said.
I just installed an amp under the drivers seat, and a sub in
the rear deck. I was shocked at how easy it all was to take
down (plastics) and the stock head unit leaves a ton of room
for running wires!
It was a pleasure to work on my box!
