I have to do a presentation in the form of a "safety meeting" for my Industrial Safety class and my topic is Material Safety Data Sheets.

I have everything completed except I need a "case study" or whatever. Basically an overall report of an accident involving MSDS's such as an incident in which there were none on hand or they had incorrect information.

I've looked all over the web, on OSHA's website, and many other places but can't find anything. If you know of any, please help!
