When I saw the pic of the lugs missing I figured there was more wrong than that, broken wheel studs like I thought, sucks. Only thing I can think of is when you shortened the lug nuts for the spacer it had a negative effect, less threads to hold the load, and snap. Run APR extended wheel studs for the IS300, hubcentric slip-on spacers, done. Drawback is wheel selection gets less, positive is you won't have things breaking down. I might be doing that and running without center caps for my Style 5's if I can't get enough threads with some open end tuner lugs I got. Since the hole for my wheels I had redrilled is just enough to run McGard spline drives, the only other lugs that might work are Muteki. Won't know until I get my car fixed though.

Hammer those _____es out man I have to replace all my rears because the same ____ happened while running spacers. It's because the wheels are heavy, not the right center bore, and tuner lugs. Tuner lugs will actually loosen them self over time they suck...good thing your rim didn't fly off...you already know my story haha.

fixed it yesterday. bought these
realized the heads were different
tried installing it anyways, and it was a millimeter too big and caught onto the hub
so i shaved it down
to this
and i bought some new lugnuts and repainted them to blue. weee!!!

Some wheels won't mount or balance properly so you have to mount them backwards depending on the machine. Not always the case..he could have been stupid but I've run into wheels before that required me to mount it backwards in order to balance.
