Monroe Bearing Plate - defects..or?

Hey guys,
I recently switched my front suspension to a new set of monroe sensa tracs and the bearing mount plates. Soon after a week of driving my passenger shock starts clanking at every bump, so Its lookin like a blown strut.
I ordered a replacement for this week, but I also noticed what might be something wrong with the bearing plate.
Looks like part of the plate is seperated, and almost looks as if its floating and I remember it being flush on the stock ones. Pics attatched...
Any comments? Are they defects too perhaps?

Scion/Toyota Manual states that the nuts are a one time use and each time you take the strut or top hat out/off you must replace them. Also if the nut came loose the key-way in the top hat (the strut rod is flat on 2 sides to fit key-way) may have come out causing the clanking sound when going over a bump. If it goes unfixed it will destroy the top hat.
