Default 2006 Scion xB with 138k miles "skipping/jumping"

The past few days, I have felt this type of skipping/jumping whenever I am stopped at a stop light and begin to accelerate. I thought maybe it was due to having the A/C on and cranking but I had it off during one of the other times it had happened. Sometimes it actually feels as if my tires are slipping on oil, others my car actually feels like it jumps off the ground.

Today, I was actually stopped at a stoplight and with my foot on the break, my car felt as if it was jumping forward (but I actually wasn't moving any).

Any ideas what this could be caused by??

Note- I have had to replace 3 coil packs in the past and at first it started out like those had, but they always tripped the 3 dash lights and now it's not feeling the same.
