Sorry about the link- it stopped working, I guess. Anyway, on the Pep Boys site you can search for item 30103. It's called a Tapa-Circuit With Dual Fuse Holder. I think.
I've seen it on a few other pages (JC Whitney, for example), but PepBoys seems to be the cheapest- just over $7.
